6/14/2023 EnchantaVaughn Approved Behavior: Upon arrival, to the home setting Ben-Nita appeared to be in a positive mood. QMHP-C engaged mother in a discussion to determine her behaviors since the last session. Mother reports Ben-Nita continues to struggle with managing her anger. Behavior: During the second portion of the session, Ben-Nita displayed hyperactive behaviors evident by her starring at the walls when discussing ways to resolve conflict. Intervention: QMHP-C encouraged Ben-Nita to stay in the house and do something special with her family. QMHP-C encouraged Ben-Nita to be the example. QMHP-C encouraged Ben-Nita not to follow others that she sees are doing wrong. QMHP-C intervened to assist in explaining behaviors to Ben-Nita. QMHP-C reflected with Ben-Nita on her day, both happy and frustrating experiences. QMHP-C encouraged Ben-Nita that tomorrow is a new day. QMHP-C processed Ben-Nita current feelings. QMHP-C engaged in active listening. QMHP-C employed positive reinforcement when Ben-Nita was able to achieve all of her objectives and encouraged her to maintain her positive behavior by providing her with a review of her objectives. QMHP-C educated Ben-Nita with relaxation strategies such as deep breathing on a regular basis. Intervention: QMHP-C used appropriate behavior modeling by reviewing situations which exhibits positive behavior to decrease Ben-Nita’s impulsive behavior and encourage her to use self-control. QMHP-C monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP-C provided individual and family counseling based on specific treatment objectives. QMHP-C educated Ben-Nita about using good manners at home and offering alternative methods and suggestions to model appropriate language that Ben-Nita should use to ask for things or to get someone’s attention. QMHP-C facilitated a family discussion to talk about common mistakes children with ADHD make and how she can make better choices. QMHP-C assisted Ben-Nita with recognizing her own personal strengths and how to apply them to reach her goals and objectives on a weekly basis. Response: Ben-Nita communicated the do's and don'ts of home/session rules so that she shows more commitment to her choices. Ben-Nita was in a pleasant mood after feedback was given. Ben-Nita was seen having a good time. Ben-Nita was able to identify the role she plays in each situation and continues to comply with authority rules and regulations. Ben-Nita remembered to apply herself and utilize her strengths to focus on meeting the goals listed on her ISP. Ben-Nita was acceptive of encouragement and decided to listen to her mother and do the right thing. Ben-Nita was seen following desired rules and expectations without requiring redirection or additional breaks. Ben-Nita had one on one with QMHP-C. Ben-Nita realized her mother was trying to help her by pushing her to try on her own first and she would correct her mistakes. Response: Ben-Nita was respectful of others and careful not to cross personal boundaries and space. Ben-Nita focused on things she is good at and loves to do at home in order to stay motivated in therapeutic setting and make something out of herself. Ben-Nita showed her gratitude for being able to release physical tension stating that she feels relaxed and able to catch her breathe again. Ben-Nita stated that she can be good and told QMHP-C that she will do her best to follow directions and stay attentive to the activity. Ben-Nita was able to cooperate and comply to rules and regulations due to positive influence from her family. Ben-Nita has presented herself in a responsible way and is making progress towards her goals and objectives. Ben-Nita maintains good etiquette by sitting in her seat during instructed activity, raising her hand and asking appropriate questions, being respectful toward mental health professional, remaining quiet during quiet time and active during social time, listening to her counselor, and modeling appropriate behavior.
